- Appointments can be made by calling the office Monday through Friday from 8:00AM to 5PM. If you are a new patient, or if you are here for a CDL physical, FAA flight physical, Respirator or Hazmat physical, please arrive at least 15 minutes early to fill out paperwork. Our providers do their best to stay on time, but if you are late for your appointment all other patients after you will have to wait. If you are 15 minutes late you may be asked to reschedule.
- When you come for your appointment please bring your insurance card and a list of all the medications you are taking.
- The amount of time scheduled for you appointment is usually long enough to manage one health concern. This is why we ask what you need to be seen for. If you have several concerns, you may be asked to schedule an additional appointment so that we can address each concern thoroughly.
- If you miss an appointment you may be charged.
